
FEES AND PAYMENTS
We
make every effort to
keep down the cost of medical care. You can help by paying
upon completion of each office visit. Payment may be made by
cash, check or Visa/Mastercard. Other arrangements can be
made depending on special circumstances.

Dr. Eldridge is a
Medicare Participating Physician. We accept assignment for
all services provided to Medicare patients. Please remember
that Medicare does not pay for normal eye examinations or for the
refraction part of your eye examination. We will file your
Medicare claim and Medicare will pay us 80% of the approved charge,
after subtracting any part of the yearly deductible you have not
met. After Medicare pays us, the claim is usually submitted
to your secondary insurance to collect the remaining 20% of the
approved charge. We will then bill you for the part of the
yearly deductible you may have not yet met and for any coinsurance not
paid. You will also be charged for the refraction part of the
examination, which Medicare does not cover.
Insurance and
Insurance Forms
Please pay any
copay that your insurance requires at the time of your visit.
It is your responsibility to pay any copayment, deductible amount,
coinsurance or any other balance not paid for by your insurance
company.
